1. Obtain the appropriate forms from the company managing your account. Forms are generally available on the company website or by calling the customer service number on your statement to request a form be mailed to you.
2. List Ocean Rescue Alliance as a beneficiary. Use our Tax Identification Number, 83-4231286, where the form asks for a Social Security Number.
3. Submit a form and notify ORA about your gift so we can thank you.
